What a DEXA Scan Actually Measures (And Why Your Scale Can’t Tell You)

You step on the scale. It says 178 pounds. Same as last month.

So the last four weeks were a waste, right? The 5 a.m. sessions, the meal prep, the protein you forced down when you weren’t hungry – all of it, apparently, for nothing.

Except that number is one of the least informative measurements you can take about your body. It cannot tell the difference between four pounds of fat and four pounds of muscle. It cannot tell you whether the fat you are carrying is sitting harmlessly under your skin or wrapped around your liver. And it has nothing at all to say about your bones.

A DEXA scan can answer all three questions. Here’s what it actually does.

The Short Version of the Science

DEXA stands for dual-energy X-ray absorptiometry. The scanner passes two low-energy X-ray beams through your body at different energy levels. Fat, lean tissue, and bone each absorb those beams differently – and because the two beams are absorbed at different ratios by each tissue type, the software can work backward from what came out the other side and calculate how much of each tissue is present, pixel by pixel.

That last part matters. A DEXA scan doesn’t produce one number for your whole body. It maps composition by region – left arm, right arm, trunk, left leg, right leg, and more. That’s how you find out that your right quad is carrying noticeably more lean mass than your left, nine months after the knee surgery you thought you’d fully rehabbed.

That matters less as a brand name and more as a reference point: it means your results sit on the same measurement scale as the research literature you might read about body composition.

The Four Numbers Worth Knowing

1. Body fat percentage – and where it lives

Total body fat percentage is the number most people come in for. It’s useful. But regional distribution is where the insight usually is. Two people can both land at 24% body fat and have completely different metabolic risk profiles depending on where that fat is stored.

2. Lean mass, limb by limb

Lean mass is muscle, organs, and water. Tracking it is the only reliable way to know whether a fat-loss phase is actually preserving the muscle you spent years building – which is the entire ballgame if you want to keep your strength and your metabolic rate intact.

It’s also how you catch asymmetries. Runners, lifters, and anyone rehabbing an injury routinely discover a 5–10% difference between limbs they had no idea was there.

3. Visceral adipose tissue (VAT)

This is the one that surprises people.

Visceral fat is the fat packed in and around the organs in your abdominal cavity. It is metabolically active in a way subcutaneous fat isn’t – it secretes inflammatory signaling molecules, and elevated levels are associated with insulin resistance, cardiovascular disease, and type 2 diabetes. You can be lean-looking and still carry a concerning amount of it. The term for that is “normal weight obesity,” and a bathroom scale will never flag it.

DEXA estimates visceral fat directly. It’s one of the few accessible ways to put a number on it outside of an MRI or CT.

4. Bone mineral density

DEXA is the clinical gold standard for measuring bone density – it’s what physicians use to diagnose osteopenia and osteoporosis. Your report includes T-scores and Z-scores, the standardized values your doctor already knows how to read.

Most people don’t think about their bones until something breaks. But bone mass peaks somewhere around age 30 and declines from there, and the decline accelerates sharply for women through perimenopause and menopause. A baseline in your 40s is worth considerably more than a first scan at 65, because a single reading tells you where you stand – while two readings five years apart tell you which direction you’re heading and how fast.

How It Compares to What You’re Probably Using Now

Bathroom smart scales and handheld devices use bioelectrical impedance, which sends a small current through your body and estimates composition from the resistance it encounters. The problem is that the current follows the path of least resistance – usually water – so your readings swing with hydration, sodium intake, when you last ate, and whether you just trained. Same-day readings can vary by several percentage points.

Skinfold calipers can be decent in the hands of a very experienced practitioner testing the same person repeatedly. Most of the time they aren’t, and they say nothing about visceral fat or bone.

DEXA has a precision error typically under 1–2% for body composition, and it isn’t meaningfully affected by hydration status. That precision is what makes repeat scans trustworthy: when your visceral fat drops between scans, you can be confident that’s a real physiological change and not measurement noise.

No method is perfect – DEXA has its own known limitations, including sensitivity to positioning, which is why we standardize how you’re positioned every single time.

What the Appointment is Actually Like

Shorter and less eventful than most people expect.

Before you come in: Wear clothes without metal – no zippers, underwire, buttons, or grommets. Leave the jewelry at home or plan to remove it. Come consistently hydrated rather than unusually hydrated. If you’re pregnant or think you might be, don’t schedule a scan; tell us, and we’ll help you find a better option.

During: You lie on your back on a padded table, fully clothed, while an arm passes slowly above you. Nothing touches you. Nothing is enclosed. The scan itself takes roughly six to ten minutes, and the hardest part is holding still.

On radiation: The dose from a body composition DEXA scan is extremely small – a tiny fraction of a chest X-ray, and comparable to the natural background radiation you’d absorb over a few ordinary hours. Ask your technician for the specifics on our scanner if you’d like the exact figure.

After: You get your results the same day, and a technician walks you through them before you leave. You shouldn’t need a background in exercise physiology to understand your own body.

How often should you rescan?

For most people pursuing a body composition goal: every three to four months. Real, measurable change in lean mass takes that long to accumulate, and scanning more often mostly buys you noise and anxiety.

For bone density tracking, the interval is longer β€” typically one to two years, depending on your risk factors and what your physician recommends.

Try to keep conditions consistent between scans: similar time of day, similar hydration, similar time since your last meal and last hard training session.
